TOPIC: Chest Infections TYPE: Medical Student/Resident Case Reports INTRODUCTION: Bronchogenic cysts are congenital malformations found in children and adults. Although they the most common primary of mediastinum, can be anywhere respiratory tract. For children, account for 5-10% mediastinal masses. In adults, typically asymptomatic incidentally on chest imaging. CASE PRESENTATION: A 40 year old male with past medical history significant paroxysmal atrial fibrillation presented pain dysphagia. He was being evaluated a mass that discovered CT underwent endobronchial ultrasound bronchoscopy (EBUS) 1 week prior to this subsequent encounter. Cytology from EBUS showed laden macrophages some benign columnar cells. Repeat interval growth 4.7 cm x 4.2 4.4 8 6 7 cm. At point, patient converted into rapid ventricular response, thought result effect right atrium. Interventional radiology consulted guided biopsy during which 180 ml pus extracted sent cytology sheets neutrophils. Following procedure, developed severe became hypotensive requiring norepinephrine. urgent video-assisted thoracoscopic surgery (VATs), placement two tubes, bronchogenic cyst visualized. DISCUSSION: Cough, dyspnea, symptoms patients cysts. Less include dysphagia as seen our patient. Majority complications compression adjacent structures. As enlarges, it lead bronchial obstruction leading air trapping distress. Infection is complication especially communication. These become infected septic shock.
